Early Transition

Transition from Early Intervention to Preschool Programs includes activities that enhance the smooth and effective movement from early intervention programs to preschool programs, school, other related services, or exit of the program.  This includes the development and implementation of an Individualized Education Plan (IEP) by the child’s third birthday.  All elementary schools provide pre-k speech and language services for children age 3-5 within their zone. Additionally, preschool special classes are available.

School Age Transition

Transition for school age students occurs at the natural transition points – those periods that are close in time to the transition of a child with a disability from:

  • Preschool to Elementary

  • Elementary to Middle School

  • Middle school to High School

  • High School to Post-Secondary School Activities

Transition activities for school age students include collaboration of staff from sending and receiving schools and IEP team meetings to develop Individualized Education Plans.

Post Secondary Transition

Students in High School are required to have Transition Plans developed to address preparation for employment and independent living .  As part of some students plans, they participate in Work-Based Learning Programs that enable them to acquire job skills and develop employability knowledge.  This incorporates a curriculum developed to teach students employment and other post-school adult living objectives with the acquisition of daily living skills and functional vocational evaluation.
