Mutual of Omaha Long Term Disability Benefits Insurance

Long Term disability benefits are available to full-time employees of Hamilton County Department of Education. The policy is provided at no cost to the employee.

If you become disabled, by either accident or illness, you may be a candidate for income replacement and extension of your group life insurance.

The Long Term Disability policy has an elimination or waiting period* of 180 calendar days. After the disability elimination period is met the policy pays 66 2/3% of your basic monthly earnings. This monthly benefit will coordinate with other disability benefits you may receive so that the total benefit does not exceed two-thirds of your salary. You will be required to apply for all benefits for which you may be entitled.

*Elimination period means a period of continuous total or partial disability which must be satisfied before you are eligible to receive benefits. Benefits are not paid during the elimination period.

In the event you become disabled contact the Benefits Department at (423) 209-8566 to request an application. The application process may be started once you have been off the job and disabled for four months. If approved, the monthly benefit begins after you have satisfied the 180 calendar day elimination period.

When you request paperwork to apply for long term disability you also will receive an application to apply for life insurance “waiver of premium”. Once completed, you mail the long term disability application to Mutual of Omaha and the life insurance waiver application to AIG Life Insurance Company.

In the event an employee establishes disability under Mutual of Omaha, individual medical insurance will remain in force for 12 months at no cost to the employee (not applicable to terminated employees). An employee who leaves employment as disabled under the terms of the Tennessee Consolidated Retirement System will continue to be covered under the individual medical insurance for an additional 18 months for a total period of board paid individual coverage not to exceed 30 months from the date of disability retirement or the effective date of Medicare whichever is earlier. This extension of coverage (from 12 months up to 30 months) will be at no cost to the employee. The cost of dependent coverage will be the responsibility of the employee.
