Pre-Kindergarten Application Information
What you will need to register your child in a pre-k program.
To apply, parents need to bring the following:
- Original birth certificate - (Child must be 4 years old by September 30)
- Social security card
- Current immunization record
- Current physical record
- Proof of residency
- Proof of income
Tennessee State Immunizations Required for Pre-Kindergarten:
- Diphtheria/Tetanus/Pertussis (DTP/DTAP/DT/TD) 4
- Haemophilus (HIB) 3 or 4
- Polio (OPV/IPV) 3
- Measles/Mumps/Rubella (MMR) 1
- Hepatitis B 2 or 3 series
- Varicella (CPX) 1 or date of chicken pox history
Physical Exam ( Must be done within one year of registration)
Upon entrance into kindergarten, parents whose children attend a pre-kindergarten program must register at their zoned elementary school.
