What documents are needed to register a new student?

• 2 (two) Proofs of Residence
  
  • Lease/Rental agreement/Mortgage.
  • Real estate tax receipt
  • Public assistance/government benefits
• Parents Driver’s License or State Issued ID
• Most Recent Utility Bill (within the last 30 days)
• TN Immunization Certificate (state-required immunization & must be up to date)
• Birth Certificate (preferred) but not required
• Proof of physical within the past 12 months on a TN medical form.
• Social Security Card (preferred)
