Wellness Plan

Engaging with the Wellness Plan is straightforward and rewarding. The HCS Wellness Plan involves getting your yearly free physical at one of our HCS Clinics or your own primary care physician. At our clinics you might also receive recommendations for additional wellness coaching. Utilizing HCS pharmacies and completing a confidential and HIPAA-compliant annual wellness survey are other simple ways to engage, and while getting your annual flu shot is encouraged, it's not mandatory.

Employees who fully engage in our Wellness Plan will see a beneficial impact on their medical benefit rate and other offerings. Employees who do not engage with the Wellness Plan and/or our smoking cessation program (as applicable) may see cost increases reflected in their monthly medical premiums. This plan is all part of our effort to encourage a healthier, more vibrant HCS community.

Your Path to Wellness Compliance

 

  1. You must have obtained a physical between November 1, 2023 - October 31, 2024. Have your provider complete the Physical Form and upload it to the wellness portal.
    • Upload the completed physical verification form to the WellRight Portal before November 7, 2024. Physicals may be obtained for free at the EdHealth & Wellness clinic or from your primary care physician
  2. Utilize the EdHealth Pharmacy for any maintenance/routine medications.
    •  Exemptions apply and may be approved by Benefits.
  3. Complete the Health Risk Assessment found on theWellRight Portal.
  4. Optional: Annual Flu Shot is recommended but not required.

Important Dates

Nov. 1, 2023 - Oct. 31, 2024

 
  • Complete the online health risk assessment.
  • Complete annual physical
  • Call EdHealth Pharmacy to discuss maintenance prescriptions or note in your wellness portal that no maintenance medications are taken.

November 7, 2024

 
  • Deadline for uploading the Annual Physical form.

Who Needs to Engage?

Employees are the only individuals who will need to engage in the HCS Wellness Plan. Retirees, spouses, and dependents do not need to engage in the Wellness Plan.
