Short Term Disability Insurance from Symetra. Certificate of Coverage for those enrolled in the plan. 

Short term disability benefits are available to full-time employees of Hamilton County Department of Education. A base policy that pays 60% of your basic weekly earnings up to a maximum of $500 per week is provided at no cost to the employee.  An additional buy up policy is also available by payroll deduction. The buy up policy increases the benefit to 66 2/3% of your basic weekly earnings up to a maximum of $2,000. 

The Short Term Disability policy has an elimination or waiting period* of 7 calendar days.  If you have more than 7 days of eligible sick and personal leave, short term disability benefits do not become payable until all sick and personal leave are exhausted.  The maximum disability period is 180 days.  This weekly benefit will coordinate with other income you may receive such as retirement or other work earnings.

*Elimination period means a period of continuous total or partial disability which must be satisfied before you are eligible to receive benefits. Benefits are not paid during the elimination period.
